In the role I envision for him, he won't be needed early in the season anyway but I would probably insist on our own doctors checking him out before signing him. If they say "operate" and he refuses, I probably would pass. If our doctors say "just rest" or they say "operate" and he agrees to the operation, I'd probably be good with signing him and then PUPing him. Like I said, he's not going to be pricey and he's good insurance if healthy but there's no need to sign him if he can't be ready to go eventually.
